Hyaluronic acid is a natural substance produced by the human body in abundance among young people. Its role in the skin is to attract moisture into it. This makes it an appealing substance for injection as a dermal filler to help minimize fine lines and wrinkles.
One aspect of fillers that is appealing is that hyaluronic acid fillers are temporary and reversible.
Hyaluronidase is an enzyme able to dissolve hyaluronic acid fillers if an error has been made by a practitioner or if the patient is unhappy with their results for any reason.
Other reasons why Hylenex may be used include:
- Nodularity
- Vascular compromise emergency
- Contaminated filler
- Blindness emergency
Although many types of hyaluronidase exist, the type that we use is Hylenex because it is available in a form that is least likely to result in an allergic response. Reversing fillers using Hylenex is not really an exact science. The amount required to dissolve a hyaluronic acid filler can vary greatly. Some considerations include:
Fillers that are strongly cross-linked are more difficult to dissolve.
The longer that a filler has been in the body, the harder it is to dissolve.
In emergency situations, higher doses are needed.

Receiving Hylenex
Hylenex treatments are normally very comfortable, lasting less than five minutes. Lidocaine can be mixed with the Hylenex to enhance patient comfort, numbing the area for about 30 minutes. Lidocaine is a safe substance used in every hospital in America. The hyaluronic acid should be dissolved within a few hours, sometimes minutes. It may take a day or two for post-ejection swelling to resolve itself. If the patient would like to have their filler replaced, depending on the area, it can be administered in three days to a max of 6 weeks apart. If further treatment is necessary, follow-up injections can be delivered in three days or later.
